要求
首先，假條的上方還是應(yīng)該按照半正式的格式寫上如下信息：
To：假條是遞給誰的
From：請假人
Date：寫假條的日期(注意不是請假的日期)
Subject：寫上請假字樣
其次，在您的請假信第一段，應(yīng)該開門見山但是有禮貌地提出請假。第一段要中心明確，寫清您要請假的日期。
然后，在假條的第二段，您應(yīng)該簡單明了陳述請假的原因事由。注意敘述清楚明白，并盡可能表示對此帶來工作不便的歉意。（比如原因就寫打球后回家洗澡感冒。）
接著，在假條的最后一段，應(yīng)寫上您希望獲得準(zhǔn)假的句子，或者具體等候答復(fù)的時間。

首先，要在假條的上方寫明假條是遞給誰的

To：假條是遞給誰的

From：請假人

Date：寫假條的日期（注意可不是請假的日期，不要弄混了哦！）

Subject：寫上請假字樣

例如：
